Why, why, why, why, why…

Our Principal Consultant, Dave Porrill, is a busy man, last week he was running some training programmes at the Vitol site, VPI Immingham

The training was split into 2 courses, morning and afternoon, repeated over 3 days. The morning session was entitled Root Cause Analysis’, the afternoon ‘the 5 Why Process’, which is a simpler method for failure analysis at ‘shop floor’ level.

During the morning sessions, the delegates spent time designing a template that they will use across the site for tracking and monitoring their formal Root Cause analyses. In industry, this type of template is often referred to as an ‘A3’.
